PROOF OF RESIDENCY IS ONLY REQUIRED IF YOUR ID DOES NOT INDICATE OHIO RESIDENCY. Proof of residency must be dated within the past 30 days. Proof of residency has to be an official document (not handwritten), it must include your full name, it must include your current address, and there must be a clearly marked date from within the last 30 days.
